Patient had broken external fixation bar so (under general anesthesia) the bar was removed and a new one was put in place. Additionally the IM pin was cut at the junction to the old bar and the end of the pin was covered in foam tape and wrapped up along with the new bar in cast padding, pool noodle foam, and vet wrap.

The areas of the patagium and suture site at the distal carpus were covered as well.
Bandage change. The vetwrap, foam padding, and cast padding was removed. The pin tracts were clean and dry with no erythema or swelling. The pin tracts were cleaned using dilute betadine and saline, and the external fixator device rewrapped with cast padding, foam padding, and vetwrap. The carpal bumper on the distal right wing was replaced, and had to be reapplied again after it came off putting 081 back in his run.

Pin placement under anesthesia via Isofluorane. The external bar was removed from the pins using bolt cutters. The IM ulnar pin was rotated back out of the ulna by 3cm to allow room for placement of the new ESF pin. A 1.6mm pin was chosen and placed distally. External tubing was placed over all 3 pins and filled with acrylic gel. Once the gel had hardened, the distal ESF pin was cut down to the level of the tubing using pin cutters. The wound on the right carpus was debrided and 3 simple interrupted sutures were placed with 4-0 PDS. The tubing was wrapped with bandage material, foam tubing placed over that, and vet wrap as the most external layer. The wing was placed in a wing to body wrap using silk tape. Patient recovered uneventfully.

Patient was anesthetized for laser therapy and physical therapy. Anesthetized via mask with Isofluorane. Starting heart rate was 230bpm and respirations were 20breaths/minute. The bandage was left intact and laser therapy was done on the inner wing at the same time as physical therapy. Physical therapy consisted of several range of motiion exercises targeting the elbow and carpal joints over the course of 5 minutes. The range of motion appeared better at the end of PT. The laser treatment lasted 8 minutes and 30 seconds at 765J over the humerus. A circular 1cm wound on the carpus was noted and closed with 4-0 PDS and two simple interupted sutures. A telfa bandage was applied over the sutures. A small bleeding wound was also noted on the beak between the nares and two layers of duoderm was applied to supply some cushioning. telfa bandages were applied over the opposite carpus as well to provide some padding. Recovery from anesthesia was smooth.

Patient was anesthetized for a bandage change, laser therapy, physical therapy, and recheck radiographs. We used Isofluorane gas to anesthetize the patient and supplemental oxygen. Once anesthetized we removed the bandage from the external tubing and pins. The skin under the bandage appeared healthy and pink. Laser therapy was performed for 8 minutes and 30 seconds at 765J and 20 Hz. Physical therapy was performed to increase mobility of the carpal and elbow joints. The wing was repeatedly extended, flexed, and rotated to improve mobility. Radiographs were taken of the right ulna. Lateral anbd CrCd views were taken. The pins were all in place and the fracture appeared to be healing well. The external bar was rewrapped using cast padding and vet wrap. The patient recovered quickly and smoothly.

Slight amount of dried blood was noted around ulnar pin site. Pin exit sites were cleaned with betadine. Telfa padding was placed around pin sites. Insulation padding was placed over ex fix bar and the wing was taped back to the belly band in a wing to body wrap.
Patient recovered normally from anesthesia.

Procedure - Fracture repair - ulna | Performed by - Amberly Sokolof | Surgery time - 0 minutes | An incision was made over the fracture and the callus formation
at the fracture was debrided. A pin was introduced on the caudal
aspect of the ulna distal to the olecrannon at a point between
the bases of the second and third secondary feathers at a near
right angle to the long axis. With a back-and-forth rotation,
the blades of the trocar were embedded into the cortex about
two-thirds of its depth. A 3/32in IM pin was introduced aligned
with the long axis while continuoing the rotations of the chuck
so that an elongated hole is cut in the bone to the level of
the fracture. The fracture was reduced and the pin was driven
towards the distal end of the bone stopping short of the end
of the ulna. A 1.6mm ESF pin was inserted in the proximal fragment
of the ulna between the olecrannon and the point of entry of
the IM pin so that the ESF pin doesn't share the medullary space
with the IM pin. A 1.1mm ESF pin was inserted in the distal fragment
so that it doesn't share the medullary space with the IM pin.
The IM pin was bent at a 90 degree angle to the bone. A plastic
tube was placed over the IM and ESF pins and filled with acrylic
that was cured for 12 minutes with the wing folded against the
body to establish rotational alignment. All pins were cut. The
incision over the fracture was closed using 5-0 PDS suture in
a simple continuous pattern. A bandage was applied over the ESF
using cast padding, tegaderm, and vet wrap. The patient recovered
uneventfully. |

Physical exam: Noted hematoma and soft tissue swelling at the distal ulna on the dorsal side of the wing between the secondary feather shafts. The callus involving the radius might still be in the fibrous stage because it is soft. No bruising noted. Patient was picking at the cast padding from the bandage, so we removed the figure 8 wrap and placed a wing-to-body using only vet wrap.
